Practicum Experience Plan

Purpose Statement

The purpose of this paper is to state my three or four objectives, planned activities, mode of

assessment, and state what am expecting as my course outcome. Knowing the course content from

the beginning is very important to me to be successful in this program because this will enable me to

set a goal and monitor my progress. More importantly, this will not only help me to be successful in

this program but will also help me to be successful in my national exam. Yusuff (2018), setting goals

and appropriate planning help students to be focused and be successful in their academic

performance. Evidence-based practice shows that personalized goal settings and study planning have

helped students to be successful not only in the classroom but also after completing their program.

Introduction

Developing personalized goals and study planning is very important for me to complete this

program successfully. Every student has a different lifestyle that can interfere with their academics

and studies, but developing objectives, planned activities, mode of assessment, and expected

outcomes will help in guiding me through what am expected to accomplish for the course and the

program at large. Aghera et al., (2018), students should not only list their goals, but they should also

execute them and that is what results in success. One of the importance of the goal is that it helps in

promoting learning performance.

Overview:

Your Practicum experience includes working in a clinical setting that will help you gain the

knowledge and skills needed as an advanced practice nurse. In your practicum experience, you will

develop a practicum plan that sets forth objectives to frame and guide your practicum experience.

practicum experience but also work through various patient visits with focused notes as well as one

Part 2: Individualized Practicum Learning Objectives

Refer to the instructions in Week 2 to create individualized practicum learning objectives that meet

the requirements for this course. These objectives should be aligned specifically to your Practicum

experience. Your objectives should address your self-assessment of the skills found in the “PMHNP

Clinical Skills Self-Assessment Form” you completed in Week 1.

As you develop your individualized practicum learning objective, be sure to write them using the

SMART format. Use the resources found in Week 2 to guide your development. Once you review

your resources, continue and complete the following. Note: Please make sure each of your

objectives is connected to your self-assessment. Also, consider that you will need to demonstrate

how you are advancing your knowledge in the clinical specialty.

** YOU MUST HAVE 3 NEW OBJECTIVES EACH QUARTER. You may include previous practicum objectives; however, you still must have 3 new objectives for your current course.

Objective 1: During this clinical rotation I planned to learn how to select the appropriate and

evidence-based clinical practice guidelines for my patient treatment plans and medication

prescriptions. I also plan to learn the appropriate informed consents that are necessary for all ages

and the required regulation for the state in which I will be practicing as a Nurse Practitioner.

Planned Activities: During my patient assessment, I planned to ask them about the medication that

they have used before and the effectiveness of the medication. I will give them varieties of options

for their treatment and let them the less restrictive one and the one that comes with the highest risk.

Mode of Assessment: I will like to assess my patient individually. Honavar (2018), the one-to-one

approach will enable the patient to tell the provider all the necessary information that is needed for

their psychiatric evaluation. Before prescribing any medication to my patient, I will make sure they

are informed about the risk and benefits, and ask for their preference, and what they can afford. This

is important because the majority of psychiatric patients are not treatment compliant because they

cannot afford the medication that is prescribed for them.

PRAC Course Outcome(s) Addressed: My professional plan is to learn and understand how the

right approach to address my patient and build a very good professional relationship with them. This

will not only make me a great Nurse Practitioner but it will help alleviate my patient’s symptoms.

This is because every patient wants to be related to their provider effectively and they want to be

able to express their feelings to a Nurse Practitioner that is willing and ready to listen to their

concerns.

Objective 2: Secondly, in this clinical rotation, I planned to learn the most appropriate screening

method and the instrument that are engage-specific after learning the appropriate screening method,

I also planned to implement and interprets them efficiently and accurately.

Planned Activities: I planned to develop an appropriate and patient-specific treatment plan of care

for my patient that are based on their screening result and response. Putting all the information

obtained from the patient together will help in developing a treatment plan that is specific to their

symptoms and illness.

Mode of Assessment: I will give my patient the questionnaire for the appropriate screening tool that

is meant for their age and diagnosis. Before handling the questionnaire to them, I will encourage

them to answer the question to the best of their ability and also let them know that any response that

they put for the questionnaire will not be used to judge them but rather be used to develop an

appropriate treatment plan for them accordingly.

PRAC Course Outcome(s) Addressed: By the end of this clinical rotation, I planned to have the

right knowledge on how to use the screening tool for my patient and effectively utilized it for their

treatment plan. I will also share the result of the screening tools with the other treatment team

members to use in developing appropriate treatment plan options for the patient can include patient

referrals and the psychotherapy approach.

Objective 3: I plan to learn the appropriate documentation that is required for proper psychiatric

evaluation including the legal and ethical standards that is ag appropriate. I also planned to know the

appropriate process for intake interviews and referral procedures. Accurate documentation promotes

patient care and helps to speed up their recovery process (Momenipur & Pennathur, 2019).

Planned Activities: I planned to learn the appropriate documentation from my preceptor and ask

questions. I also planned to put all my documentation in my patient’s own words.

Mode of Assessment: I will assess my progress by documenting patient psychiatric evaluation

during my clinical rotation and do more intake assessments and referrals.

PRAC Course Outcome(s) Addressed: By the end of this clinical rotation, I planned to have

learned effective documentation, intake assessment, and age-appropriate referrals for my patients.

Part 3: Projected Timeline/Schedule

hours and activities must be supervised by your Preceptor and completed onsite. Your Preceptor will

approve all hours, but your activities will be approved by both your Preceptor and Instructor. Any

changes to this plan must be approved.

you are making now.

I intend to complete the 144 or 160 Practicum hours (as applicable) according to the following

timeline/schedule. I also understand that I must see at least 80 patients during my practicum

experience. I understand that I may not complete my practicum hours sooner than 8 weeks. I

understand I may not be in the practicum setting longer than 8 hours per day unless pre-approved

by my faculty.

Conclusion

By highlighting all my SMART goals for the course, I believe this will help me to be

successful in the course and also for my nation examination. During the program, I believed I have

learned a lot, but are still more to cover especially to be successful in the national exam. To be

successful all the objectives, planned activities, and course outcomes must be followed promptly.

Moreover, to achieve all these, I need to incorporate discipline and good time management. To be

successful in an academic environment and life generally one needs to set a goal that is higher and

more specific, this is one of the best management tools to increase personal performance and

motivation (Höpfner & Keith, 2021).
