48 Program Evaluation For this Assignment, use the program you identified in Unit 3 and develop a logic model to demonstrate relationships between the program’s vested resources, activities, and proposed outcomes or results. Create a graphic representation of the logical model for your evaluation plan. Use the Readings, Discussion, and Seminar from Unit 4 to guide you through the model. After you have completed your model, in a 3 page paper describe the relationships represented in your model. Make sure to include: • List all inputs, program activities, program outputs, and short and long-term outcomes. • Describe the relationships between the items in bullet one. • Describe how the activities in the program are going to lead to the proposed outcomes.
